Retatrutide: −25% in phase 3, and a gray market that got there first

The phase 3 trials were published in September 2026. Long before that, it was already sold online as "research use only". Why retatrutide is at the center of the controversy.

What it is and why the hype

Retatrutide is a triple agonist: it acts on the GLP-1, GIP and glucagon receptors at once11. In phase 2 (338 adults with obesity), the 12 mg dose was associated with −24.2% body weight at 48 weeks vs −2.1% with placebo1. In type 2 diabetes, after a phase 1b multiple-dose trial12, phase 2 recorded weight reductions of up to 16.9% at 36 weeks vs 3.0% with placebo13. The number spread on social media long before any approval.

The phase 3 results

TRIUMPH-1 was published on September 29, 2026: 2,339 adults without diabetes, 80 weeks. Body weight changed −17.6%, −23.7% and −25.0% with 4, 9 and 12 mg, vs −3.9% with placebo; knee-osteoarthritis pain and sleep apnea also improved2. Gastrointestinal effects were the most common2. The trial, sponsored by Eli Lilly and Company, is registered as NCT0592906614.

TRIUMPH-2, in 1,152 adults with obesity and type 2 diabetes, associated the 12 mg dose with −18.8% body weight at 80 weeks vs −5.1% with placebo (a −13.8-point difference). Diarrhea (27–34% vs 13%) and nausea (14–28% vs 8%) were more frequent than with placebo, as were hypotension (1–6% vs <1%) and dysesthesia (abnormal skin sensations, 4–7% vs 1%)3.

The gray market got there first

While the trials were still running, retatrutide was already sold online as a "research peptide". In December 2024 the FDA sent a warning letter to Summit Research Peptides for selling semaglutide, tirzepatide, retatrutide, cagrilintide and mazdutide, concluding that evidence from its websites established the products were "drugs for human use"5. In August 2026 it sent another to Royal Peptides, whose labels said "for research use only" and "not for human or animal consumption"6.

The FDA keeps a page on unapproved GLP-1s warning against semaglutide, tirzepatide, retatrutide and other products sold "for research"4. And in August 2026 Eli Lilly sued six companies; according to Healio, Lilly says the sellers "falsely claim their drugs are for 'research-use only' when they are intended for human use"7. A 2026 medical review lists retatrutide among investigational drugs that an illicit market offers without a prescription, lacking an established human safety profile17.

What is in those vials?

We found no published analyses of retatrutide bought online. There are for semaglutide: a study that made test purchases from six illegal online pharmacies found, in the vials received, purity of 7.7% to 14.4% vs the 99% claimed, and endotoxin in every sample; the three pens ordered never arrived10. What does exist is a 2026 case report: a man with type 1 diabetes developed severe vomiting, ketonemia and acute kidney injury shortly after injecting an online-purchased product sold as retatrutide; a Shigella infection complicated attributing the cause15.

The region, and the rumor about Panama

In Colombia, INVIMA included retatrutide in its alert 184-2026 on fraudulent peptides offered on Instagram and a website8. An Argentine outlet reported in August 2026 that "countries such as Brazil, Paraguay, Colombia and Panama have already banned the sale of Retatrutide"9. Argentina did make it formal: ANMAT Disposition 6140/2026, published in the Boletín Oficial on September 24, 2026, banned the use, sale, advertising and distribution of products declaring retatrutide, except authorized clinical research16.
