Why everyone is talking about BPC-157
BPC-157 is a pentadecapeptide (15 amino acids) described as stable in human gastric juice18. Its fame started in gyms and podcasts, and in 2026 it reached politics: in an interview with Joe Rogan, US Health Secretary Robert F. Kennedy Jr. said, "I'm a big fan of peptides… I've used them myself and with really good effect on a couple of injuries"1. Critics such as biologist Paul Knoepfler answered that undoing the restrictions carries "major risks to the public"16.
What the FDA did in 2026
The FDA had placed BPC-157 in "Category 2" of bulk substances for compounding, those that may present significant safety risks. Its entry still warns that compounded BPC-157 "may pose risk for immunogenicity for certain routes of administration"3.
On April 15, 2026 the FDA announced it was removing 12 peptides from that category "due to their nominations being withdrawn by the nominators" and scheduled its Pharmacy Compounding Advisory Committee (PCAC)2. On July 23 the committee voted 8 to 6, with one abstention, to add BPC-157 to the compounding list; the use reviewed was ulcerative colitis421. As the American Med Spa Association noted, "The FDA is not required to follow the committee's recommendation"5. FDA scientists had in fact proposed that BPC-157 "NOT be included"19.
What the human evidence shows
This is where the enthusiasm runs into the data. All human evidence published in PubMed is three small, uncontrolled reports by the same lead author, from private US clinics:
- Knee pain: a retrospective review with a phone survey of 16 patients after intra-articular injection (4 of them received BPC-157 plus thymosin beta-4); 14 of 16 reported relief. No control group or validated scales6.
- Interstitial cystitis: 12 women, injections in a single cystoscopic procedure, no control group; 10 of 12 reported complete symptom resolution7.
- Intravenous "safety pilot": 2 participants, both of whom had already received IV BPC-157; two infusions on consecutive days, with no measurable effects on the markers tested through day 38.
Independent reviews reach the same conclusion. A 2025 systematic review found 36 studies, 35 preclinical and 1 clinical, and wrote: "No clinical safety data were found"9. A University of Utah review wrote that human data are "extremely limited" and concluded BPC-157 "should be considered investigational"10.
A lot of research, few labs
In animals the literature is large and consistently shows healing signals in tendon, gastric mucosa and other tissues9. But in our PubMed count (October 9, 2026), 175 of 233 BPC-157 records (~75%) list Predrag Sikirić of the University of Zagreb as an author. That is not a judgment of quality, but it does mean independent replication is thin.
Reviews from the Zagreb group mention clinical trials in inflammatory bowel disease (under the code PL 14736) in which it was safe, but no results of those trials are indexed as such in PubMed18. Independent studies exist, in animals: in rats and dogs the half-life was under 30 minutes13, and a toxicology program in mice, rats, rabbits and dogs found no genetic or embryo-fetal toxicity14.
The cancer question
In cultured human endothelial cells and in rats, BPC-157 has been associated with VEGFR2 receptor activation and new blood-vessel growth11. Because tumors depend on angiogenesis, some reviewers flag a theoretical risk; a 2026 review lists "pathological angiogenesis, carcinogenesis" among potential risks of unregulated peptides12. No human data confirm or rule out that concern.
